Nurse Case Manager: Seeking full-time Registered Nurses (RNs) to serve as Case Managers for seriously ill patients. As a case manager, you will provide skilled nursing services in a patient’s residence, which includes assessing, planning, implementing and evaluating phases of nursing care. You will work alongside your clinical and operational team members to ensure provision of high-quality hospice services. Candidates must possess excellent assessment skills, critical thinking skills as well as strong organizational abilities. Applicants must have 1-2 years of acute care experience; hospice or home care experience strongly preferred. Must have current NCBON licensing without restriction. PRN (as needed) Hospice Aide: Seeking a PRN Hospice Aid to provide excellent, professional personal care to seriously ill patients. Hospice aide services augment care given by caregivers in a patient’s residence. The hospice aide functions under the direction, instruction, and supervision of the Registered Nurse (RN). Hospice Aide must document visits and tasks completion within electronic medical record and communicate patients’ current health status and needs to the RN. Candidates must be a Certified Nursing Assistant I or II; must have reliable transportation to and from work assignments.
Full-time and Part-time Social Worker (MSW): Seeking both a full-time and a part-time social worker to provide psychosocial support for seriously ill patients and their families. Responsibilities include performing psychosocial assessments, education and completion of advance care planning, supportive counseling, and assistance with obtaining needed resources, including placement. You will work alongside your clinical and operational team members to ensure provision of high-quality hospice services. Applicants with master’s degree in Social Work preferred; will consider BSW (must be from an institution or school accredited by the Council on Social Work Education). At least one (1) year experience as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred. Download Application Here
Licensed Practical Nurse (LPN): The full-time Licensed Practical Nurse provides skilled nursing care to seriously ill patients under the direction of the RN and assists with planning and implementing nursing care for the comfort and well-being of the patient. Direct nursing care, within the scope of practice, includes but is not limited to, the following duties: screening, observing and assessing patients, providing basic wound care and personal care to patients, collecting vital signs and specimens, observing and documenting pertinent patient information, and assisting the Registered Nurse as required. Applicants must have at least one year of nursing experience; hospice/home care experience preferred. Current North Carolina LPN license.
