Admissions Team:  Seeking a full-time Registered Nurse to join our Social Worker in serving as the organization’s admissions team, positions that are part of the Trusted Triangle and interdisciplinary team.  The admissions team is often is one of the first contacts with the organization, guiding patients and their families through the initial assessment. This team plays a significant role in the educational process for patients, families and caregivers.  Primary responsibilities include processing referrals, gathering all necessary medical records and supporting documentation, and completing admissions for patients who meet eligibility criteria for hospice.  In addition, this team works professionally with referral partners, patients, families and members of the community.  Applicants must have 1-2 years of acute care experience; hospice or home care experience strongly preferred. Nurse Case Manager: Seeking full-time Registered Nurses (RNs) to serve as Case Managers for seriously ill patients. As a case manager, you will provide skilled nursing services in a patient’s residence, which includes assessing, planning, implementing and evaluating phases of nursing care. You will work alongside your clinical and operational team members to ensure provision of high-quality hospice services. Candidates must possess excellent assessment skills, critical thinking skills as well as strong organizational abilities. On-Call Nurse: We are hiring a Full-time Registered Nurse (RN) to provide on-call coverage. The Hospice On-Call Nurse answers calls from patients, families, caregivers and referral partners and triages needs, utilizing critical thinking skills. The On-Call Nurse performs patient visits, including admissions to hospice services, in private residences, skilled nursing centers and assisted living communities. This Nurse also provides symptom management advice and self-care interventions and identifies resources. In addition to being a great listener, this Nurse must be a strong communicator and have the sense to know when to ask more or less questions. Candidate must be able to exercise independent and sound judgement in order to prioritize and schedule visits based on care needs. Full-Time, Licensed Practical Nurse (LPN). The Licensed Practical Nurse provides skilled nursing care to seriously ill patients under the direction of the RN and assists with planning and implementing nursing care for the comfort and well-being of the patient. Direct nursing care, within the scope of practice, includes but is not limited to, the following duties: screening, observing and assessing patients, providing basic wound care and personal care to patients, collecting vital signs and specimens, observing and documenting pertinent patient information, and assisting the Registered Nurse as required.

Hospice Aides: We are hiring full-time, part-time, and PRN (as needed) Hospice Aides to provide excellent, professional personal care to seriously ill patients. Hospice aide services augment care given by caregivers in a patient’s residence. The hospice aide functions under the direction, instruction, and supervision of the Registered Nurse (RN). Hospice Aides must document visit and tasks completion within electronic medical record and communicate patients’ current health status and needs to the RN. Candidates must be a Certified Nursing Assistant I or II; must have reliable transportation to and from work assignments.
